Please bring Certified Birth Certificate and Shot Records.

This screening is for preschool children, birth thru 4 years that will not be going to Kindergarten in the fall.

If you are unable to attend this screening, other arrangements can be made by calling 927-6441.  Please schedule screening appointments prior to April 27^th.  The Preschool office will be closed during screening week.
